If your student has any issue health issues, whether temporary or lifelong, please connect with our health office at [email protected].

 

Any student who needs to take medication while on campus needs to have this form completed and signed by a parent/guardian on file. Paper copies are available in the Front Office. 

 

If your student has allergies, needs an epi-pen, or may go into anaphylaxic shock, please contact the Health Office so we can develop a health and emergency plan for them.
